DaVita seeks RN to manage Home Hemodialysis patients.
Train patients/caregivers to perform dialysis independently at home.
Clinical care, patient education, and case management role.
Monday–Friday 8-hour shifts, weekends and holidays off.
Flexible scheduling managing your own patient caseload.
Home visits, assessments, and on-call support required.
Reliable transportation and valid driver's license essential.
ADN required, BSN preferred for this specialty position.
Basic computer skills needed for documentation.
