Educate patients and caregivers in-clinic to perform independent home dialysis safely.
This role blends clinical care, comprehensive patient education, and case management expertise.
Manage your caseload at 909 N Topeka St with Monday–Friday 8-hour shifts only.
Conduct assessments, develop care plans, and monitor treatment progress effectively.
Perform home visits and provide on-call support while collaborating with care teams.

ADN required (BSN preferred) with valid driver's license and reliable transportation.
