Responsibilities
- Support patients across Mankato and New Ulm clinics
- Conduct psychosocial assessments for dialysis patients
- Develop comprehensive care plans for ESRD patients
- Coordinate care between multiple clinic locations
- Provide financial counseling and insurance assistance
- Offer individual and family counseling sessions
- Facilitate access to transportation and support services
- Collaborate with clinic staff across locations
- Maintain accurate documentation for all patients
- Monitor treatment adherence and barriers
