- Experience in dialysis or healthcare setting (experience)
- Valid driver's license and reliable transportation (experience)
- Ability to travel to patients' homes (experience)
- Bilingual skills a plus (experience)
- Experience with ESRD patients (experience)
Preferred Qualifications
- Kidney Care experience (experience)
- Case management certification (experience)
- Experience with Medicare/Medicaid (experience)
- Crisis intervention training (experience)
- Group facilitation experience (experience)
- Familiarity with community resources (experience)
- Previous outpatient clinic experience (experience)
Responsibilities
- Assess psychosocial needs of dialysis patients
- Develop individualized care plans
- Coordinate patient care with interdisciplinary team
- Provide counseling for emotional adjustment to dialysis
- Assist with financial planning and insurance issues
- Facilitate patient and family education
- Advocate for patient rights and needs
- Connect patients with community resources
- Monitor patient compliance with treatment
- Document all social work interventions
