- Experience with End Stage Renal Disease patients (experience)
- Valid driver's license and access to reliable transportation (experience)
- Ability to travel to patients' homes or other locations (experience)
- Bilingual skills a plus (experience)
- Experience in dialysis setting (experience)
Preferred Qualifications
- Experience working in outpatient healthcare settings (experience)
- Knowledge of Medicare and Medicaid regulations (experience)
- Training in crisis intervention (experience)
- Experience with patient advocacy (experience)
- Familiarity with ESRD financial assistance programs (experience)
- Certification in case management (experience)
Responsibilities
- Assess patients' psychosocial needs and barriers to treatment
- Develop and implement individualized care plans
- Educate patients on dialysis rights and responsibilities
- Coordinate with interdisciplinary team for comprehensive care
- Assist patients with insurance and financial planning
- Provide counseling for emotional and mental health support
- Facilitate access to community resources and support services
- Advocate for patient needs within healthcare system
- Monitor patient progress and adjust care plans
- Document all interventions and outcomes accurately
