Responsibilities
- Assess psychosocial needs of dialysis patients
- Develop and implement individualized care plans
- Provide counseling for emotional and mental health support
- Assist patients with financial planning and resources
- Advocate for patient rights and needs
- Coordinate with interdisciplinary care team
- Facilitate patient and family education
- Conduct psychosocial assessments
- Support transplant planning and preparation
- Maintain accurate patient documentation
