- Experience in dialysis or healthcare setting (experience)
- Valid driver's license and reliable transportation (experience)
- Bilingual preferred (experience)
- Ability to travel to multiple clinics (experience)
- Experience with ESRD patients (experience)

- Case management certification (experience)
- Experience with Medicare/Medicaid (experience)

Responsibilities
- Assess psychosocial needs of dialysis patients
- Develop individualized care plans
- Coordinate community resources and referrals
- Provide counseling for emotional adjustment
- Assist with financial planning and assistance programs
- Facilitate transplant education and evaluation
- Advocate for patient rights and needs
- Collaborate with interdisciplinary care team
- Conduct discharge planning and follow-up
- Maintain accurate documentation and records
