Position Overview

Diablo Valley Post-Acute is hiring a full-time Activities Assistant to join our team.

Compensation and Benefits

  • Competitive pay - $17.50

  • Full time - Monday - Friday 8am - 4:30pm

General Purpose

The primary purpose of the Activities Assistant is to assist the Activity Director in running a creative and interactive activities program. This role will adhere to current federal, state, local, and corporate standards, regulations, and guidelines to ensure an ongoing program of activities that meet residents' interests and their physical, mental, and psychosocial needs.

Essential Duties

  • Participate in planning and conducting individual, small, and large group activities.

  • Facilitate communication between employees, residents, families, support personnel, government agencies, and the public to ensure residents’ needs and interests are met.

  • Engage in community planning related to the interests of the facility and the services needed for residents and families.

  • Assist in developing the monthly activity calendar and maintaining attendance records.

  • Collaborate with the Quality Assurance and Assessment Committee to develop and implement plans of action for identified quality deficiencies as requested by the Activities Director.

  • Participate in discharge planning and assist in the development and implementation of activity care plans and resident assessments.

  • Arrange transportation for residents as needed, including outings or appointments.

  • Develop a monthly activity schedule for residents that includes outings, group activities, and in-room activities for bed-bound or isolated residents.

  • Encourage residents to engage in self-initiated activities such as hobbies, crafts, and reading, providing necessary materials (e.g., reading materials in Braille or audiobooks when appropriate).

  • Assist with assessment documentation and quarterly progress notes as assigned by the Activities Director.

  • Maintain cleanliness and order in the Activity Department.

Supervisory Requirements

  • This position has no supervisory responsibilities.

Qualifications

  • High school diploma or equivalent.

  • Preferably one year of experience in a long-term care facility.

Required Qualifications
