Job Description

Now Hiring: Admissions Nurse (RN or LPN)
South Creek Post Acute
Schedule: Monday–Friday, 11:00 AM – 7:00 PM

South Creek Post Acute is seeking a dedicated Admissions Nurse to join our team. This position plays a key role in coordinating patient admissions and ensuring a smooth transition into our facility.

Position Details:

  • Schedule: Monday–Friday, 11:00 AM – 7:00 PM

  • License: RN preferred; LPN will be considered

  • Pay Range: $40–$55 per hour, based on credential and experience

Key Responsibilities:

  • Coordinate and complete patient admissions

  • Review and verify admission documentation

  • Communicate with hospitals, families, and interdisciplinary team members

  • Ensure compliance with facility policies and regulatory requirements

  • Provide a positive admission experience for new residents

Qualifications:

  • Active RN or LPN license in good standing

  • Strong communication and organizational skills

  • Experience in post-acute, skilled nursing, or admissions preferred

  • Ability to work independently and collaboratively
