Position Summary

As a CNA at Lincoln Meadows Care Center, you will provide essential hands-on care and support to residents while maintaining dignity, comfort, and safety. You will work closely with the nursing team to deliver high-quality, resident-centered care in accordance with individualized care plans.






Key Responsibilities (Summary):
  • Assist residents with activities of daily living, including bathing, dressing, grooming, feeding, and mobility

  • Monitor and report changes in resident condition to the nursing team

  • Accurately document care, vital signs, and intake/output as directed

  • Respond promptly to resident needs and call lights

  • Support admissions, transfers, and discharges to ensure resident comfort

  • Maintain a clean, safe, and calm environment

  • Observe and report skin conditions, pressure areas, and safety concerns

  • Follow infection control, safety, and confidentiality guidelines

  • Work collaboratively with nursing and interdisciplinary teams






Qualifications:
  • Current Certified Nursing Assistant (CNA) license in good standing in the State of California

  • CPR certification preferred

  • Compassionate, dependable, and team-oriented

  • Strong communication and observational skills

  • Commitment to providing respectful, resident-focused care
