Job Description

Certified Occupational Therapy Assistant (COTA) – PRN
Starting Rate: $45/hour

Sequoia Transitional Care, a skilled nursing facility, is seeking a dedicated PRN Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. This role provides flexibility while making a meaningful difference in the lives of our residents.



General Purpose

The COTA assists the Occupational Therapist with patient-related activities and direct patient care, helping residents achieve their highest level of independence and quality of life.



Essential Duties
  • Provide therapy treatments as directed by the Occupational Therapist.

  • Record daily treatment notes and weekly progress notes in compliance with OT Board standards.

  • Follow physician treatment plans for each patient.

  • Assist nursing staff with training Restorative Aides.

  • Communicate effectively with the supervising OT and interdisciplinary team regarding patient progress and plans.

  • Participate in in-service training programs for facility staff.

  • Maintain accurate documentation including evaluations, recertifications, and discharge notes per state and payer requirements.

  • Provide caregiver training and family education to support safe discharge planning.

  • Assist in maintaining a clean and safe treatment area, reporting equipment issues promptly.



Qualifications
  • Current COTA license in the state of California.

  • Proficient in electronic documentation systems (e.g., Casamba, PointClickCare).

  • Strong communication skills with patients, families, and healthcare team members.

  • Ability to read, interpret, and apply therapy standards, policies, and physician orders.

  • Must meet ongoing state continuing education requirements.



Physical Demands
  • Frequent standing, walking, and reaching.

  • Ability to push/pull and lift up to 50 lbs frequently and 100 lbs occasionally.

  • Occasional stooping, kneeling, or crouching.

  • Visual acuity including close, distance, and peripheral vision.



Work Environment
  • Skilled nursing and rehabilitation setting.

  • Low to moderate noise levels.
