Job Description

MDS Coordinator – Marin Post Acute
Location: San Rafael, CA
Pay: $90,000–$130,000 annually (DOE)
Schedule: Full-Time

Marin Post Acute is seeking a skilled and detail-oriented MDS Coordinator (LVN or RN) to join our leadership team. If you are passionate about accuracy, regulatory compliance, and supporting outstanding resident care, we want to meet you!






What You’ll Do
  • Lead and manage the full MDS/RAI process in accordance with federal and state guidelines

  • Accurately complete assessments, care plans, and documentation

  • Collaborate with IDT members to ensure individualized, resident-centered care

  • Monitor compliance, quality measures, and reimbursement accuracy

  • Provide education and support to staff regarding MDS processes and updates

  • Work closely with administration and clinical leadership on audits, reporting, and workflow improvement






What We’re Looking For
  • RN or LVN with current California license

  • MDS/RAI experience required (minimum 1 year preferred)

  • Strong understanding of PDPM, care planning, and regulatory compliance

  • Excellent attention to detail and organizational skills

  • Strong communication and teamwork abilities
