Now Hiring: MDS Nurse / Case Manager (Full-Time)
Rock Creek Care Center - 260 Racetrack St, Auburn, CA 95603



Salary Range: $130,000 – $160,000 annually
Benefits: Medical, Dental, Vision, 401(k)
Experience Required: MDS experience

Rock Creek Care Center is seeking an experienced and motivated MDS Nurse / Case Manager to join our dedicated and collaborative team. This is a key clinical leadership role for a detail-oriented professional who thrives in a fast-paced skilled nursing environment and is passionate about delivering high-quality, resident-centered care.



About the Role

In this position, you will use your clinical expertise and critical thinking skills to coordinate care, manage MDS assessments, and ensure regulatory compliance while supporting optimal outcomes for our residents. You will collaborate closely with residents, families, physicians, therapy teams, and payers to develop and implement effective plans of care.






Key Responsibilities
  • Oversee and complete MDS assessments accurately and timely in compliance with state and federal regulations

  • Coordinate admissions, discharges, and care transitions

  • Communicate regularly with residents and families regarding care plans, therapy services, and progress

  • Monitor resident status and adjust care plans based on changing needs

  • Perform utilization review to ensure appropriate, timely, and cost-effective care

  • Coordinate with interdisciplinary teams, insurance providers, and community resources

  • Maintain accurate, thorough documentation and charting

  • Support quality assurance initiatives and regulatory surveys

  • Provide direct nursing care and clinical oversight as needed






Qualifications
  • MDS experience required

  • Bachelor’s Degree in Nursing or Social Work preferred

  • Minimum two (2) years of clinical nursing experience preferred

  • Knowledge of Medicare, Medi-Cal, and Medicaid programs

  • PCC experience preferred

  • Strong organizational, communication, and time-management skills

  • Must maintain licensure and remain in good standing with the Department of Public Health
