Victorian Post Acute is a beautiful 90-bed facility located in a great area with ample parking. We are a rehab-focused team that serves both short-term and long-term patients. We are currently seeking a passionate and motivated Occupational Therapist to join our dynamic in-house therapy team.

Key Responsibilities
Develop and implement effective treatment plans in coordination with referring physicians.
Provide direct therapy services following the approved plan of care.
Train and support Restorative Aides within the nursing department.
Supervise Occupational Therapy Assistants (OTAs) in patient care and documentation per state practice guidelines.
Collaborate with supervisors and interdisciplinary team members to monitor progress and adjust treatment as needed.
Participate in care conferences, utilization reviews, and rehabilitation meetings as required.
Maintain accurate and timely documentation, including evaluations, daily notes, progress reports, and recertifications per regulatory standards.
Educate patients, families, and caregivers on home programs and discharge planning to promote continued independence.
Recommend and coordinate the use of durable medical equipment (DME) to support safe mobility and function (e.g., braces, walkers, grab bars).
Report equipment issues promptly to ensure a safe and functional work environment.
Qualifications
Licensure: Valid Occupational Therapist License (State of California)
