Job Description

Physical Therapist (PT) – Full-Time
Dublin Post Acute Care – Dublin, OH

Dublin Post Acute Care is a skilled nursing and long-term care facility dedicated to exceptional resident care, cutting-edge therapy programs, and a supportive team environment. We are seeking a compassionate and motivated Physical Therapist (PT) to join our growing therapy team!

What You’ll Do:

  • Provide high-quality physical therapy evaluations and treatments for residents requiring rehabilitation in a SNF/LTC setting

  • Develop personalized treatment plans to increase mobility, function, and overall independence

  • Collaborate closely with our interdisciplinary care team including OT, ST, Nursing, and Social Services

  • Educate residents and families on care plans, safety, and progress

  • Document patient progress in accordance with state and federal regulations

What We’re Looking For:

  • Current and active Ohio Physical Therapist license required

  • Prior SNF/LTC experience preferred, but not required

  • Strong communication, clinical judgment, and teamwork skills

  • Passionate about improving resident quality of life through dignified and results-driven care
