Job Description

Physical Therapist (PT) – Skilled Nursing Facility
$60–$70/hour
Midtown Oaks Post Acute | Sacramento, CA

Midtown Oaks Post Acute is hiring a Physical Therapist (PT) to join our dynamic rehab team in a skilled nursing/post-acute care setting. We are looking for a compassionate, motivated clinician who values quality care and teamwork.

Pay Range:

  • $60–$70/hour, based on experience

Schedule Options:

  • Full-time

  • Part-time

  • PRN

  • Flexible scheduling available

Physical Therapist (PT) Responsibilities:

  • Evaluate and treat patients in a skilled nursing/post-acute setting

  • Develop and implement individualized treatment plans

  • Document evaluations, progress notes, and discharge summaries accurately and timely

  • Collaborate with nursing, therapy, and interdisciplinary teams

  • Educate patients and families on therapy goals and home exercise programs

Physical Therapist (PT) Requirements:

  • Active California Physical Therapist license (or pending)

  • Skilled nursing or post-acute experience preferred, but not required

  • Strong communication and time-management skills

  • Team-oriented, compassionate, and patient-focused
