Physical Therapist – In-House Therapy Program
Location: Rossmoor Post Acute
Hourly Rate: $57 - $67 (depending on experience)

Job Description

  • Treatment Planning: Develop effective treatment plans and obtain physician approval for services.

  • Patient Care: Provide treatment in accordance with established physician treatment plans.

  • Training Support: Assist the nursing department in training Restorative Aides and supervise Physical Therapy Assistants in patient care activities.

  • Communication: Maintain clear communication with supervisors and healthcare team members regarding patient progress and challenges.

  • Meetings Participation: Engage in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as required.

  • Documentation: Accurately record evaluations, daily treatment notes, weekly progress notes, and recertification documentation in accordance with PT Board requirements.

  • Adjustments: Document treatment changes following established policies and procedures.

  • Family Instruction: Educate patients’ families and nursing staff on maintenance programs and caregiver training for discharge preparations.

  • Discharge Planning: Contribute to discharge planning processes for patients.

  • Equipment Recommendations: Recommend and facilitate the ordering of necessary durable medical equipment to enhance patient mobility.

  • Equipment Maintenance: Report any issues with department equipment to ensure proper functioning.

Required License or Certification

  • Valid Physical Therapist License
