Nestled on Country Club Drive in Palm Desert, Willow Springs Healthcare Center provides 24/7, person‑centered skilled nursing and rehabilitation in a warm, friendly environment—with comfortable rooms, a fully equipped rehab unit, and activities that support the social and emotional wellbeing of our residents. • Lead restorative programs that make a daily difference—ambulation/walking with assistive devices, bed mobility, transfers, ADLs, ROM (AAROM/PROM), sit‑to‑stand practice, strengthening and exercise support, and restorative dining initiatives.
• Chart progress clearly in PCC/Point of Care and support the RNA Weekly Summary process with the licensed nurse attestation.
• Collaborate with Rehab & Nursing to initiate/modify restorative plans (OT/PT/ST referrals), activate tasks in POC, and keep care plans current.
• Join the interdisciplinary RNA meeting (at least monthly) to review programs, documentation, and outcomes—operating with RN oversight and a team-first approach.
• Spot changes early (pain, mobility, positioning, swallowing) and escalate appropriately to keep residents safe and progressing.

• Active California CNA certification; RNA experience/training preferred.
• Comfort with PCC/POC documentation and restorative workflows.
• A coach’s mindset—positive energy, great communication, and the ability to motivate residents kindly and consistently.
• Reliability, teamwork, and an eye for safe, person‑centered care.
