• Perform Comprehensive Assessments: Use standardized tools to evaluate housing needs, financial barriers, family strengths, and risk factors, tailoring interventions accordingly.
• Create and Monitor Service Plans: Develop actionable Housing Stability Plans (HSPs) with measurable goals, reviewing and adjusting them quarterly to promote self-sufficiency.
• Facilitate Housing Placement: Guide clients through rental searches, lease negotiations, application processes, and securing move-in funds or security deposits.
• Coordinate Holistic Referrals: Connect families to essential services like CalFresh benefits, Medi-Cal enrollment, childcare, job training, mental health counseling, and substance abuse treatment.
• Champion Client Advocacy: Represent clients in meetings with property managers, Section 8 coordinators, legal aid, and shelter providers to resolve disputes and secure approvals.
• Maintain Meticulous Documentation: Enter data into HMIS (Homeless Management Information System) and internal databases, ensuring compliance with HUD reporting standards and grant requirements.
• Execute Field-Based Support: Conduct bi-weekly home visits, accompany clients to appointments, and host community-based case reviews for safety and progress monitoring.

Required Qualifications

To succeed as an ICMS Housing Navigator, you'll bring:
