  • Coordinate Holistic Referrals: Connect families to essential services like CalFresh benefits, Medi-Cal enrollment, childcare, job training, mental health counseling, and substance abuse treatment.
  • Champion Client Advocacy: Represent clients in meetings with property managers, Section 8 coordinators, legal aid, and shelter providers to resolve disputes and secure approvals.
  • Maintain Meticulous Documentation: Enter data into HMIS (Homeless Management Information System) and internal databases, ensuring compliance with HUD reporting standards and grant requirements.

Required Qualifications
