The Care Manager is responsible for care coordination, discharge planning, and ensuring continuity of care for patients in the inpatient behavioral health setting. This includes assessing patient needs, collaborating with an interdisciplinary team, facilitating discharge planning, and providing education and support to patients and their families.
A Bachelor’s degree in Nursing is strongly preferred, but an Associate Degree in Nursing (ADN) is required if a Bachelor’s degree is not obtained. Candidates must also have a current, active, and unencumbered Registered Nurse (RN) license with the Minnesota State Board of Nursing. Social Workers with appropriate licensure may also be considered.
The position is a full-time, Monday through Friday day shift role, offering a consistent 40-hour workweek with no nights or weekends.
