Required Qualifications

• Graduate of nationally accredited nursing program (CCNE, ACEN, NLN CNEA) (experience)
• Bachelor's degree in nursing preferred (experience)
• Minimum one year of professional nursing experience preferred (experience)
• Unencumbered RN license in the State of Minnesota or Nurse Licensure Compact (NLC) state (experience)
• Maintains required department-specific competencies and certifications (experience)
• Valid driver's license (depending on location) (experience)
• Basic Life Support (BLS) certification required (experience)
• Certificate for Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) preferred (experience)
• Knowledge of nursing principles and practices (experience)
• Proficiency in patient assessment and care planning (experience)
• Ability to work independently and as part of a team (experience)
• Excellent communication and interpersonal skills (experience)

Responsibilities

• Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide professional nursing services to patients/residents at home.
• Document human responses to actual or potential health problems in the home health environment.
• Practice dependent, interdependent, and independent nursing functions.
• Collaborate with patient/resident and family to plan, implement, and evaluate care.
• Collaborate with other inter-professional colleagues, such as physicians, to plan, implement, and evaluate care.
• Demonstrate competency and practice within the full scope of nursing expertise and knowledge.
• Utilize appropriate age and population-specific standards in assigned setting.
• Care for patients/residents in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up in the home setting.
• Implement the nursing process, coordination and continuum of care, patient/resident assessment, and patient/resident education.
• Perform triage and various other nursing interventions.
• Function within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure.
• Adhere to the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient vital signs and report any changes to the physician.
• Provide wound care and other specialized nursing services.
• Educate patients and families on disease management and prevention.
• Coordinate with other healthcare providers to ensure continuity of care.
• Maintain accurate and complete patient records.
• Participate in quality improvement activities.
• Adhere to all safety regulations and infection control policies.
• Respond to emergency situations and provide appropriate care.
• Conduct comprehensive patient assessments to identify their physical, psychological, and social needs in the home environment.
• Develop and implement individualized care plans based on patient assessments and physician orders.
• Evaluate the effectiveness of care plans and make necessary adjustments to achieve desired patient outcomes.
• Provide skilled nursing care, including medication administration, wound care, and catheter management.
• Educate patients and their families on disease processes, treatment options, and self-care techniques.
• Coordinate with other healthcare professionals, such as physicians, therapists, and social workers, to ensure holistic care.
• Monitor patients' progress and report any changes in their condition to the physician.
• Maintain accurate and up-to-date patient records.
• Adhere to all applicable laws, regulations, and ethical standards.
• Participate in continuing education and professional development activities.
• Provide emotional support and counseling to patients and their families.
• Advocate for patients' rights and ensure they receive the best possible care.
