The LPN in MDS/LTC plays a critical role in assessing residents, developing care plans, collecting data for the MDS, and coordinating care with the interdisciplinary team.
An associate’s degree in nursing, a current LPN license, and experience working with the senior population are required. RAC-CT certification is preferred or must be obtained within 6 months of hire.
Key responsibilities include resident assessment, care planning, data collection, coordination of care, documentation, and direct nursing care.
Opportunities for advancement include specialized certifications, leadership roles, continuing education, and mentorship.
The estimated annual salary range is $50,000 to $75,000, depending on experience and qualifications.
