Required Qualifications

• Graduate from an accredited practical nursing program as a Licensed Practical Nurse (LPN). (experience)
• Valid and unencumbered Licensed Practical Nurse (LPN) license with the South Dakota State Nursing board or possess multi-state licensure privileges. (experience)
• If position requires IV therapy/management skills in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing. (experience)
• Basic Life Support (BLS) certification for health care providers required. (experience)
• Valid driver's license may be required depending on the location. (experience)
• One year of professional nursing experience preferred. (experience)
• Ability to perform testing to assess data reflective of the patient's/resident's status. (experience)
• Ability to interpret information needed to identify each patient's/resident's requirements relative to age-specific needs. (experience)
• Competency in practicing within the full scope of nursing expertise/knowledge. (experience)
• Utilization of appropriate age and population-specific standards in the home care setting. (experience)
• Knowledge of nursing principles for patient/resident care. (experience)
• Knowledge of medications and their effects on patients/residents. (experience)
• Knowledge of patient/resident education principles to develop a self-care program. (experience)
• Knowledge of common safety hazards and precautions to establish a safe environment, specific to the home setting. (experience)
• Ability to administer a variety of treatments and medications as directed. (experience)
• Ability to maintain required department-specific competencies and certifications. (experience)

Responsibilities

• Participate in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting.
• Demonstrate knowledge of nursing principles for the patient/resident in the relevant setting.
• Perform testing to assess data reflective of the patient's/resident's status.
• Interpret information needed to identify each patient's/resident's requirements relative to age-specific needs.
• Provide care needed based on age and population-specific standards in the home care setting.
• Ensure the health, comfort, and safety of patients/residents.
• Administer a variety of treatments and medications as directed.
• Monitor and record patient vital signs and other relevant health data.
• Document patient care accurately and completely in electronic health records (EHR).
• Educate patients and their families on disease processes, medications, and treatment plans.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Maintain a safe and clean environment for patients.
• Adhere to infection control policies and procedures.
• Provide compassionate and supportive care to patients and their families during end-of-life care.
• Assess patient's condition and report changes to the appropriate healthcare professional.
• Assist patients with activities of daily living (ADLs) as needed.
• Participate in care planning and team meetings.
• Maintain patient confidentiality.
• Follow all Sanford Health policies and procedures.
• Respond to patient emergencies and initiate appropriate interventions.
• Manage medical equipment and supplies.
• Provide wound care as directed.
• Administer oxygen and other respiratory treatments.
• Monitor and manage pain levels in patients.
• Provide emotional support and counseling to patients and families.
• Assist with advanced directives and end-of-life planning.
• Maintain professional licensure and certifications.
• Participate in continuing education and professional development activities.
