- 7:30 AM - 9:00 AM: Visit assigned patients to assess their current condition, administer medications, and provide personal care.
- 9:00 AM - 10:00 AM: Document patient interactions, vital signs, and any changes in condition in the electronic health record (EHR).
- 10:00 AM - 11:00 AM: Communicate with the Hospice team, including physicians, nurses, and social workers, to coordinate patient care.
