Required Qualifications

• High school graduate or general equivalency diploma (GED) preferred. (experience)
• Minimum age of 16. (experience)
• Prior clinical or nursing assistant experience in long-term care preferred. (experience)
• Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing. (experience)
• State certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. (experience)
• CNA training and certification required and provided by facility. (experience)
• Satisfactorily completes yearly department-specific competency testing. (experience)
• Maintains a record of continuing education credits for each year. (experience)
• A minimum of 12 hours of continuing education are required annually. (experience)
• Maintains all department-specific required certifications. (experience)
• When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry. (experience)
• Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver’s license. (experience)
• Must be medically qualified to perform the essential driving functions of this position as per our Sanford Driving Policy per the leaders request. (experience)
• Ability to read, write, and communicate effectively in English. (experience)
• Basic computer skills for documentation and training purposes. (experience)
• Ability to work independently and as part of a team. (experience)
• Ability to follow oral and written instructions. (experience)
• Physical ability to perform required duties, including lifting, bending, and standing for extended periods. (experience)

Responsibilities

• Provides assistance with basic health care needs, including daily living activities such as bathing, toileting, grooming, dressing/undressing.
• Obtains and records vital signs accurately and promptly.
• Provides psychosocial support and personal care to assigned residents.
• Assists residents in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment.
• Provides range of motion and passive exercises as directed by the care plan.
• Completes tasks related to food preparation and dining, creating a positive dining experience for the resident.
• Assists residents with meals and snacks, providing water and supplements as appropriate.
• Aids providers and nursing staff members with procedures, if needed.
• Documents resident interactions as needed and the outcomes related to resident care, behavior, activity, and dietary intake.
• Communicates resident's changing condition and care-related concerns/responses to the charge nurse.
• Collects non-invasive body fluid specimens and gathers vital signs.
• Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational.
• Provides assistance and documentation with wellness and recreational programs and encourages residents to participate.
• Follows infection prevention and control policies and Personal Protective Equipment use.
• Participates in departmental/organization committees and task forces as needed.
• Maintains a safe and clean environment for residents.
• Responds to resident calls and requests promptly and courteously.
• Monitors residents for changes in condition and reports observations to the nursing staff.
• Respects residents' rights and privacy.
• Participates in resident care planning conferences.
• Assists with admission, transfer, and discharge of residents.
• Maintains confidentiality of resident information.
• Adheres to facility policies and procedures.
• Attends and participates in staff meetings and training sessions.
