- Demonstrated active involvement in at least 100 Dermatology procedures in the last 24 months (if not applying directly out of residency) (experience)
- Residency or fellowship log (if applying directly out of residency) (experience)
- Quality profile from hospital(s) showing clinical activity for the past 24 months (if not applying directly out of residency) (experience)
