- Successful completion of a Family Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or the Royal College of Physician and Surgeons of Canada. (experience)

- Residency procedure log and a list of completed rotations (if newly completed residency) (experience)
- Quality profile from hospital(s) showing clinical activity for the last 12 months (if not applying directly out of residency) (experience)
- Case log including 25 vaginal deliveries, 50 adult medical patients, and 10 inpatient care of children under the age of 18 (excluding normal newborn), as applicable (experience)
- Demonstrated active involvement in the clinical aspects of Family Medicine (experience)
- Ability to perform history and physical examination (experience)
- Ability to admit patients (if applicable to the setting) (experience)
