Required Qualifications
- M.D. or D.O. degree (experience)
- Successful completion of a Family Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or the Royal College of Physician and Surgeons of Canada (experience)
- Board Eligible/Board Certified in Family Medicine (experience)
- Experience demonstrated through residency procedure log and completed rotations, or quality profile from hospital(s) showing clinical activity for the last 12 months (experience)
- Case log including 25 vaginal deliveries, 50 adult medical patients, and 10 inpatient care of children under the age of 18 (excluding normal newborn), as applicable to privileges being requested, in the last 12 months (experience)
- Certification in the primary area of practice by the appropriate specialty/ subspecialty board of the American Board of Medical Specialties (ABMS) (experience)
- Valid medical license in the state of Minnesota or eligibility to obtain one (experience)
- Ability to perform history and physical examination (experience)
- Ability to admit patients (if applicable to the setting) (experience)
- Ability to evaluate, diagnose, treat, and provide consultation to patients of all ages (experience)
- Knowledge of a wide variety of illnesses, diseases, injuries, mental health conditions and functional disorders (experience)
- Experience in pre-operative care of surgical patients (experience)
- Experience in post-operative medical care of surgical patients and return to work issues (experience)
