- M.D. or D.O. degree (experience)

- Case log including 25 vaginal deliveries (if applicable) (experience)
- Case log including 50 adult medical patients (experience)
- Case log including 10 inpatient care of children under the age of 18 (excluding normal newborn), as applicable to privileges being requested, in the last 12 months. (experience)
- Ability to perform history and physical examinations (experience)
- Ability to evaluate, diagnose, and treat patients of all ages (experience)
- Valid medical license in the state of Minnesota or eligibility to obtain one. (experience)
