Required Qualifications

  • M.D. or D.O. degree (experience)
  • Successful completion of a Family Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or the Royal College of Physician and Surgeons of Canada. (experience)
  • For recent residency graduates, provide residency procedure log and list of completed rotations. (experience)
  • For experienced physicians, provide a quality profile from current hospital(s) showing clinical activity for the last 12 months, including procedure numbers, morbidity, mortality, infection rates, and other complications. (experience)
  • Demonstrate active involvement in clinical aspects of Family Medicine. (experience)
  • Case log including 25 vaginal deliveries (if applicable) (experience)
  • Case log including 50 adult medical patients (experience)
  • Case log including 10 inpatient care of children under the age of 18 (excluding normal newborn), as applicable to privileges being requested, in the last 12 months. (experience)
  • Ability to perform history and physical examinations (experience)
  • Ability to evaluate, diagnose, and treat patients of all ages (experience)
  • Valid medical license in the state of Minnesota or eligibility to obtain one. (experience)
  • Board certification or eligibility for board certification in Family Medicine. (experience)
