As an RN Case Manager in Hospice, you will be the central point of contact for patients and their families, coordinating all aspects of their care. Your day as an RN Case Manager will be diverse and dynamic. You might start by visiting a patient at their home, assessing their current condition, and adjusting their medication as needed. You will then collaborate with the hospice team to update the patient's care plan. A Bachelor's degree in Nursing is strongly preferred, graduation from an accredited nursing program, a minimum of two years of clinical experience, and a current unencumbered RN license in Minnesota or multistate licensure if in a Nurse Licensure Compact (NLC) are required.
