Role Overview

  • Plan, organize, direct, and evaluate activities of the professional and supportive nursing staff engaged in resident plan of care.
  • Evaluate care provided to each resident and keeps care plans current.
  • Direct lead in regards to the Resident Assessment Instrument (RAI) process.
  • Assist in assessment and evaluation of potential admissions.
  • Complete resident assessments upon admission, quarterly, and annually.
  • Collaborate with the resident, family or advocate, other inter-disciplinary colleagues, including providers, to assure ongoing care of each resident to provide the best quality of life possible.
  • Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care to patients.
  • Demonstrate competency and practices within the full scope of nursing expertise/knowledge.
  • Utilize appropriate age and population-specific standards as designated in their assigned clinical setting.
  • Care for residents in all phases of preventative care, health maintenance, diagnosis, and treatment.
  • Responsible for the coordination of care, resident assessment, resident education and various other nursing interventions.
  • Serve as charge nurse and delegate appropriate tasks to licensed and unlicensed assistive personnel that are within the job descriptions, skills, resident care needs, and the competence level of the delegate.
  • Provide adequate assistance and support to delegates to ensure safe, reliable administration of resident care and resolution of care and service concerns.
  • Function within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure.
  • Adhere to the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.
  • Conduct and document comprehensive resident assessments in accordance with federal and state regulations.
  • Develop and implement individualized care plans based on resident assessments.
  • Monitor and evaluate the effectiveness of care plans and make revisions as needed.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated care.
  • Provide education and support to residents and their families.
  • Participate in quality improvement activities to enhance resident care outcomes.
  • Ensure compliance with all applicable federal, state, and local regulations.
  • Maintain accurate and complete documentation of resident care.
  • Supervise and mentor nursing staff.
  • Participate in staff meetings and training sessions.
  • Respond to resident and family concerns in a timely and professional manner.
  • Maintain a safe and clean environment for residents and staff.
