ACUTE INPATIENT SOCIAL WORKER

HEART FAILURE PROGRAM

SUMMARY:

Provides advocacy, education, assessments and interventions to patients and families in the inpatient hospital setting to achieve an optimal level of care. Coordinates with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning to achieve quality clinical outcomes.

RESPONSIBILITIES:

  • Licensed Clinical Social Worker (LCSW) required as defined by their primary work state (Idaho or Oregon). Those colleagues hired before January 1, 2020 must provide licensure or proof of application in process in their secondary state by August 28, 2020.  Individuals hired in this role after January 1, 2020, must provide licensure or proof of application in their secondary state by August 28, 2020 or within six months of hire, whichever date is later. Master’s Degree in Social Work required.

  • Two years of experience in hospital setting or medical social work setting preferred.

  • Experience in case management, addiction services, mental health, or population health management desired.

WHAT YOU WILL DO:

Provide clinical supervision to LMSW’s pursuing a LCSW. Assess and diagnose patients with psychological conditions; understand and apply the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Collaborate with providers and psychiatrists to develop comprehensive treatment plans. Independently administer extensive psychotherapy interventions to assist with earlier discharge and/or to avoid psychiatric placement. Assess patients to apply treatment modalities, such as but not limited to, Psychoanalytic/Psychodynamic Therapy, Dialectical-Behavioral Therapy, Cognitive-Behavioral Therapy, and Relational Therapy. Manages ethical issues, diagnoses, safety concerns, and best practices in clinical interventions. Participates in care coordination activities for those patients identified as needing social work and/or complex case management interventions. Works collaboratively with healthcare team members to efficiently allocate resources. Accurately documents assessments, plans, and interventions in the EMR. Responsible for coordination of care to mental health and/or substance abuse facilities. Responsible for following established patient choice policy. Responsible for coordinating with physicians and other legal entities relating to, but not limited to, guardianships, involuntary psychiatric holds, power of attorney, and advanced directives. Collaborates with physicians and hospital staff through formal and/or informal multidisciplinary case conferences to meet the emotional, mental, physical and spiritual needs of patients and/or their families/caregivers Responsible for weekend and holiday social work coverage. Actively participates in department staff meetings and other teams or committees as required.
