Social Worker for Mount Carmel East, Night Shift

*This position will have a rotating schedule of Sunday - Tuesday and Monday - Wednesday; 7:00 pm - 7:30 am. There is a weekend rotation of 1 weekend day every two weeks.

Position Purpose:

The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from Multi-disciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team).

Minimum Requirements:

· Education: Masters degree in Social Work

· Licensure / Certification: Current LSW license to practice as a social worker in the State of Ohio

· Experience: Minimum 1 year experience in discharge planning in an acute care setting or an ambulatory setting.

· Effective Communication Skills

· Strong organization skills, self-directed, flexible, able to adapt to rapidly changing regulatory requirements.

· Possesses excellent clinical assessment and analytical skills.

· computer competency

· Possesses effective interpersonal skills, can work across departmental boundaries, facilitates problem resolution, and maintain a professional demeanor in difficult situations.

· Knowledge of the functional operations of utilization management, discharge planning, care coordination and transitions of care

· Understanding of pre-acute and post-aute venues of care and post-acute community resources.

· Ability to work independently and exercise sound judgement in interactions with physicians, payers, colleagues and patient and their families.

· Maintains a professional demeanor
