Hospice Admission Nurse (RN)


Work a 3‑Day Week — Get Paid for 40 Hours!

Friday–Sunday | 9:00 AM – 10:00 PM

Covering Livingston, Oakland and Washtenaw Counties. Paid mileage.

Work 36 hours and still receive full‑time pay, benefits, and stability — plus four days off each week to recharge, spend time with family, or enjoy life outside of work.

We’re looking for a compassionate, highly skilled Hospice Admission Nurse (RN) who is committed to providing exceptional care and supporting patients and families during one of life’s most important transitions.



About the Role

As a Hospice Admission Nurse, you’ll use your clinical expertise and strong communication skills to complete comprehensive assessments, establish individualized plans of care, and help patients and families feel informed, supported, and comforted. You’ll work closely with an interdisciplinary team to ensure seamless, high‑quality care from the very first visit.

You’ll also play a key role in the admissions process — providing education, completing documentation, coordinating with schedulers, and supporting ongoing team needs.



What You’ll Do
  • Complete thorough physical, emotional, social, and spiritual assessments for patients and families
  • Collaborate with physicians, the interdisciplinary group (IDG), hospice leadership, and caregivers to develop personalized plans of care
  • Communicate proactively with providers and team members to ensure smooth coordination of care
  • Document accurately and in real time (documenting in the home and syncing after each visit)
  • Participate in admissions activities including informational visits, consents, PROGs, discharge planning, and full admission visits
  • Provide education and support to patients and caregivers based on learning needs and condition
  • Assist with patient visits to Routine, GIP, or Continuous Care patients when admissions volume is low
  • Contribute to quality improvement, peer reviews, huddles, and ongoing clinical initiatives
  • Maintain knowledge of agency policies and state/federal regulations
  • Participate in orientation, training, and ongoing education
  • Serve as a resource and functional supervisor for LPNs and Hospice Aides


What You Bring
  • Graduate of an accredited Nursing program
  • Active Registered Nurse (RN) license in the state of practice
  • Minimum 1 year of professional acute care nursing experience or Hospice
  • Hospice or home health experience preferred
  • Strong communication and interpersonal skills — especially in grief/loss situations
  • Valid driver’s license and reliable transportation
  • Ability to demonstrate commitment to the organization’s mission and Code of Ethics
  • Ability to adapt in a fast‑changing environment with frequent prioritization shifts
  • Ability to lift 35 lbs and perform the physical demands of patient care
