In the afternoon, you may travel to a local nursing home to provide care to residents, managing chronic conditions, addressing acute illnesses, and ensuring their overall well-being. You'll collaborate with nursing staff and other specialists to coordinate care and optimize patient outcomes. Throughout the day, you'll document patient encounters in the Meditech EMR system and communicate with other members of the healthcare team.
