WVU Medicine is seeking a dedicated and compassionate RN Case Manager to join our team at Princeton Community Hospital (PCH). In this role, you will play a crucial part in ensuring patients receive the right care, at the right time, and in the most appropriate setting. As an RN Case Manager, you will be responsible for comprehensive planning for targeted patient populations, resource management, utilization management, discharge planning, care facilitation, and referral to other levels of care. The minimum qualifications include a current Registered Nurse (RN) license in West Virginia or an eNLC multi-state license and three (3) years of clinical experience.
A Bachelor’s degree in Nursing (BSN) is preferred but not required.
