Required Qualifications

• Current Registered Nurse license issued by the state of West Virginia or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC). (experience)
• Valid driver’s license. (experience)
• CPR certification within 30 days of hire date. (experience)
• One year of Home Care Nursing experience (preferred). (experience)
• Two years of prior nursing experience (preferred). (experience)
• Ability to perform the functions of a hospice aide. (experience)
• Proficiency in basic patient care according to policies and procedures. (experience)
• Knowledge of providing a safe and comfortable environment in the patient’s home. (experience)
• Understanding of patient privacy and dignity in performing all patient care. (experience)
• Ability to complete assignments in a timely manner. (experience)
• Availability for on-call responsibility per policy and procedures. (experience)
• Effective communication skills to report patient care problems to the Administrative Staff and/or other IDT members. (experience)
• Competency in providing care based on physical, psychosocial, communication, safety, education level, and related criteria for the infant, pediatric, adolescent, adult, and geriatric patient. (experience)
• Knowledge of admission criteria per agency guidelines. (experience)
• Ability to formulate and establish a plan of care with the IDT based on assessed findings and update under physician orders. (experience)
• Proficiency in completing and documenting full system assessments, including assessment of level of physical and psychosocial aspects and identifying age-specific criteria upon admission and each visit thereafter. (experience)
• Understanding of pathophysiologic processes and the ability to take appropriate nursing action in accordance with changes in patient condition. (experience)
• Ability to report significant observed changes in patient condition to Administrative Staff and/or physician. (experience)
• Proficiency in documenting patient status and care or services furnished in appropriate areas defined by the documentation policy and procedure. (experience)
• Ability to document hospice aide supervisory visits as per standard. (experience)
• Competency in performing CPR when directed by patient’s appropriate representative’s wishes. (experience)
• Ability to safely, efficiently, and prudently use and maintain D.M.E. and medical supplies and adaptive equipment. (experience)
• Proficiency in demonstrating bag technique per policy and procedure. (experience)
• Ability to apply and maintain equipment according to the Policy and Procedure Manual. (experience)
• Ability to utilize supplies effectively to ensure cost containment. (experience)
• Proficiency in maintaining the integrity of the IV site while performing patient care. (experience)
• Ability to perform treatments/procedures according to specific standards. (experience)

Responsibilities

• Provide direct and indirect nursing care to patients in their homes.
• Act as case manager, collaborating with the patient, family, and health care team (IDT) to develop, implement, and evaluate nursing care according to agency and system policies.
• Perform the functions of the hospice aide as needed.
• Monitor and perform basic patient care according to established policies and procedures.
• Maintain a safe and comfortable environment for patients in their homes.
• Ensure patient privacy and demonstrate concern for their dignity in all aspects of care.
• Complete assignments efficiently and in a timely manner.
• Assume on-call responsibilities as per agency policy and procedures.
• Communicate patient care problems or potential issues to the Administrative Staff and/or other IDT members to ensure continuity and coordination of care.
• Provide age-specific care based on the physical, psychosocial, communication, safety, and educational needs of patients.
• Adhere to admission criteria as per agency guidelines.
• Collaborate with the IDT to formulate and establish a comprehensive plan of care based on assessment findings, updating the plan under physician orders.
• Conduct and document thorough system assessments, including physical and psychosocial evaluations, identifying age-specific criteria upon admission and each subsequent visit.
• Understand pathophysiologic processes and take appropriate nursing actions in response to changes in patient condition.
• Report significant changes in patient condition to the Administrative Staff and/or physician.
• Document patient status and care services accurately and thoroughly in accordance with documentation policies and procedures.
• Document hospice aide supervisory visits as per established standards.
• Perform CPR when indicated and directed by the patient's wishes or representative.
• Promote the safe, efficient, and prudent use and maintenance of Durable Medical Equipment (DME), medical supplies, and adaptive equipment.
• Utilize bag technique in accordance with policy and procedure.
• Apply and maintain equipment following the guidelines in the Policy and Procedure Manual.
• Effectively utilize supplies to ensure cost containment.
• Maintain the integrity of IV sites while providing patient care.
• Perform treatments and procedures according to specific standards and guidelines.
• Provide comprehensive patient education on disease processes, medications, and self-care techniques.
• Act as a patient advocate, ensuring their rights and needs are met.
• Collaborate with other healthcare professionals to coordinate and optimize patient care.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
• Maintain professional competence through continuing education and professional development activities.
• Adhere to all ethical and legal guidelines in the provision of nursing care.
• Provide emotional support and counseling to patients and their families.
• Assist patients and families with end-of-life planning and decision-making.
• Coordinate hospice services with other community resources.
• Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as needed.
• Provide bereavement support to families following the death of a patient.
• Ensure compliance with all applicable regulations and standards of practice.
• Maintain accurate and up-to-date patient records.
• Participate in the development and implementation of policies and procedures.
• Serve as a resource for other members of the healthcare team.
