As a Social Worker in our Hospice program, you will be an integral part of an interdisciplinary team dedicated to providing holistic care to patients with life-limiting illnesses. You will conduct comprehensive assessments, develop individualized care plans, and provide counseling and support to patients and families to address their emotional, social, and spiritual needs. Your work will involve connecting patients and families with community resources, assisting with advance care planning, and advocating for their rights and wishes.

The minimum qualifications include a Master’s Degree in Social Work from an accredited school of Social Work, current licensure to practice Social Work in West Virginia, a valid driver’s license, and CPR certification within 30 days of hire date.
Preferred experience includes two (2) years of Hospice experience, experience with counseling individuals or groups, and experience with psychosocial, emotional, and spiritual assessment.
Core responsibilities include completing comprehensive assessments, developing hospice plans of care, assessing for abuse and neglect, assisting with advance directives, referring patients to community resources, providing counseling and support, and participating in interdisciplinary team meetings.
